PAYMENTS
Payment is due upon session, unless treatment sessions have been previously purchased .e.g. gift certificate, voucher.
Cash, CashApp, Venmo preferred.
Visa, MasterCard, Discover and American Express are accepted methods of payment. There is an additional business tax added to each service.

This form, along with the waiver & release form, must be signed & completed prior to any treatment performed.
APPOINTMENT REQUESTS & RESERVATION FEE:
ALL APPOINTMENTS ARE HELD WITH A CREDIT CARD. Online booking will show you if there is pre-payment or not.
COSMETIC TATTOO APPOINTMENTS ARE RESERVED WITH A $50 Non-Refundable RESERVATION FEE (per PMU service).
Reservation fee is required. This holds your appointment!
Please understand, I am blocking 2-3 hours just for you, canceling last minute can not be filled.

Invoice must be paid within 24 hours or the requested appointment can not be held.

CANCELATIONS WITHIN 24 HOURS:
We require 24hours notice for Cancelation / reschedule of your reserved services.
Canceling less than 24 hours will result in loss of reservation fee.
Loss of Reservation fee can be avoided by rescheduling within the same week.
System sends confirmation & Reminder texts 48 hours prior to your reservation.

CANCELATION POLICY:
24 hours notice required for cancelation or reschedule. If within 24 hours a 25% charge may apply. no-call/no-show WILL be charged 100% of service booked.
